Optimal Home Health Care, Optimizing Home Health in the San Luis Valley is dedicated to work as a team, to promote quality patient-centered care to the people of the San Luis Valley, and to provide excellent service to all agency customers in their homes in a fiscally responsible manner.
Purpose and Scope
Optimal Home Health Care functions as a Licensed Home Health and Community Support agency providing multi-disciplinary-, short- and long-term care on an intermittent and part-time basis to patients of all ages, to include adults and geriatric patients.

Services provided directly include Skilled Nursing and Home Health Aide services, Physical Therapy, Occupational Therapy and Medical Social Work per physician referral.
This agency does not provide Hospice Care, Psychoactive Care of Volunteer Services.
Services are provided to patients with medical and surgical diagnoses.
Services are available on 24-hour basis seven days a week as deemed necessary by the patient’s condition.

Assesses patient needs and develops plan of care with physician. Coordinates and oversees care plan with patient, family and support staff. Monitors and evaluates patient response to treatment, medication, healing or mobilization progress, and manages cases.

Provides hands-on support with daily activities such as bathing, dressing, grooming, mobility, and light assistance to help patients remain safe and comfortable at home.
