Home Health Care

Optimal Home Health Care functions as a Licensed Home Health and Community Support agency providing multi-disciplinary-, short- and long-term care on an intermittent and part-time basis to patients of all ages, to include adults and geriatric patients. 

Services provided directly include Skilled Nursing and Home Health Aide services, Physical Therapy, Occupational Therapy and Medical Social Work per physician referral.

This agency does not provide Hospice Care, Psychoactive Care of Volunteer Services.

Services are provided to patients with medical and surgical diagnoses. 

Services are available on 24-hour basis seven days a week as deemed necessary by the patient’s condition.

 1:      To ensure that all patients are provided with quality, comprehensive health care services through a coordinated Plan of Care.

2:      To provide continuity of health care to facilitate uninterrupted service within the health care system. 

3:      To promote early recovery of maximum rehabilitation in the patient’s home environment.

4:      To provide effective channels of communication with the patient, physician and other organizations and providers within the health care community. 

5:       To foster professional and personal satisfaction of all employees by maintaining a positive environment that stimulates pride and professional growth. 

6:       To increase the awareness of health-related issues to members of the community.
